The failed back syndrome: the diagnostic contribution of computed tomography.

چکیده
Up to 30% of patients who undergo surgery for lumbar disc disease experience recurrent symptoms postoperatively.1 2 This so-called 'failed back syndrome' is a complex phenomenon influenced by organic, psychological, economic and social factors.3,4 Radiological examination is frequently requested in order to exclude-an anatomical basis for the patient's complaints3 (Table 1). Re -exploration in the presence of significant epidural scarring (fibrosis) from previous surgery may lead to further more severe scarring,5 although some surgeons may consider microsurgical lysis of scar tissue (DP Byrnes, personal communication). Neural compression due to recurrent disc protrusion or canal or lateral recess stenosis may be relieved surgically.',6 Accurate radiological diagnosis of these and other possible abnormalities is therefore of paramount importance,'17especially in view of the difficulties in performing re-exploration of the lumbar spine.

Dose Assessment in Computed Tomography Examination and Establishment of Local Diagnostic Reference Levels in Mazandaran, Iran
Background: Medical X-rays are the largest man-made source of public exposure to ionizing radiation. While the benefits of Computed Tomography (CT) are well known in accurate diagnosis, those benefits are not risk-free. CT is a device with higher patient dose in comparison with other conventional radiation procedures. Objective: This study is aimed at evaluating radiation dose to patients from ...
متن کاملPost-operative Pseudomeningocele after Spine Surgery: Rare Cause of Failed Back Syndrome
Background and Importance: Pseudomeningocele is a rare complication of spine surgery, and it is the collection of cerebrospinal fluid in paraspinal tissues. Giant pseudomeningoceles are still rare, and very few cases have been reported in literature. It is usually occult in presentation, and patients do not have any symptoms ascribable to it. Case Presentation: We came across two sym...
متن کاملDiagnostic Reference Levels for Computed Tomography Examinations in Iran: A Nationwide Radiation Dose Survey
Introduction: International Commission on Radiological Protection introduced three basic principles of radiation protection, namely justification, optimization, and dose limit. Medical exposure has no dose limits, and generally, diagnostic reference levels are used as a tool for optimization of patient protection. Material and Methods: Dosimetry was performed on 20 CT scanners located in 14 cit...
متن کاملDiagnostic Reference Level of Computed Tomography Examinations and Need for Dose Optimization in Ondo State, Nigeria
Introduction: The present study was conducted to obtain State diagnostic reference levels (DRLs) of five routine computed tomography (CT) examinations from two CT centers in Ondo State and to identify factors responsible for dose variation and escalation in these CT centers. Material and Methods: Acquisition parameters and CT dose indices wer...
متن کاملEstablishment of Diagnostic Reference Levels and Estimation of Effective Dose from Computed Tomography Head Scans at a Tertiary Hospital in South Africa
Introduction: Head scans are the most frequently performed computed tomography (CT) examinations worldwide. However, there is growing concern over the probability of increased cancer risks among the exposed populations. Diagnostic reference levels (DRLs) identify radiation dose that is not commensurate with clinical objectives. The aim of this study was to establish DR...
